Forms:
Change of Program

Each master's program and/or concentration may have its own professional, personal, scholastic, and other standards for graduate study, including qualifying examinations. All admitted graduate students must submit a Graduate Change of Program form in order to change master's programs and concentrations/options/tracks within the program. Requests to obtain this form should be directed to the Office of Admissions and Students Recruitment, Attention: Graduate Admissions.

Note:
If you are a graduate student who is trying to change from one program to another and have already submitted a Graduate Application you must fill out a Graduate Student Change of Program Application. Do not fill out another CSUMentor Graduate Application as this may result in assigning you two different student identification numbers. The Graduate Student Change of Program Application fee is $55.00 because you are essentially applying to a new program and will therefore require a new evaluation of your file and an admission decision from the new program for which you are seeking admission.

It is critical that you indicate on the CSUMentor Graduate Application the correct program(s) you are interested in; including a masters degree and credential program or masters degree and certificate. Please read through each section of the application and you will find the appropriate choices to select to indicate your program(s) choices. You cannot apply through CSUMentor to two different masters degree programs with one application for the same term.
